FLOTILLA 12-42 MEMBERS:

The following are three important Auxiliary electronic forms that each flotilla member should complete for timely reporting of information to AUXDATA.

Form 7029H - MEMBER ACTIVITY LOG - Updated March 2012

This form is used to report all activities not reported on other AUXDATA or Mission forms. Include study time for classes, travel time to meetings, preparation of FSO reports, etc. Click on this link for the PDF/paper version.  Complete this form following the included instructions.

An on-line Electronic Submittal version of the 7029H form is now available by clicking here.  Instructions on how to use this form are on-line with the form.  The web form differs from all other Auxiliary reporting methods in that you may use any web browser - anywhere, anytime - to enter your personal data into a national database.  And you can use the system each day, like a diary, or enter your data in bulk, at the end of each month.  It is highly recommended that you now use this on-line version of the 7029H form in place of the paper/PDF form above.

Form 7030 - MISSION ACTIVITY REPORT - Updated March 2012

This email form is used to report all missions, such as public education classes and on-the-water patrols under orders. When more than one member participates in a mission, only the Lead Member should complete the form. Click on this link above and complete the form, entering the FSO-IS email address and your and/or the lead member's email address, as appropriate.

Form 7029/7030 COMBINED Member/Mission Activity Form - Updated April 2012

This is a form that combines the PDF/paper 7029 and 7030 in to one form.  It is an Excel
spreadsheet form that you save as a PDF and submit to your FSO-IS via either paper or email.  This is a specialized D11S form and will not be used by the majority of members. Get it here.
